I acknowledge that participating in sports activities at All Access Sports involves inherent risks including, but not limited to, the risk of serious injury or death. I voluntarily assume all risks associated with my participation.

I hereby release, waive, discharge, and covenant not to sue All Access Sports, its owners, officers, employees, and agents from any and all liability, claims, demands, and causes of action arising out of my use of the facility.

I agree to follow all facility rules and guidelines, and understand that failure to do so may result in removal from the premises without refund.

I consent to receiving emergency medical treatment if necessary and agree to be responsible for any medical expenses incurred.
